We are a team of licensed independent social workers, case managers, and coordinators that offer high-quality mental health services. We support members as a mental health collective by offering counseling, managing medications, coaching, and other services. We assist our patients in locating a doctor who can best suit their needs and is available by phone or in person.

Our Services
Diagnostic Assessment
This time will be spent evaluating your physical and mental health to provide medication to alleviate mental health symptoms.
Psyctherapy session
This is designated for individuals to process and explore their thoughts. Typically, meetings are weekly or bi-weekly.
Case mangement
Online or in-person, this session will help you with what you care most about and develop a plan to achieve personal and professional goals.
Community & Home health services
Our goal is to provide person-centered care that supports individuals in their own homes or communities. This includes enhancing access to care, promoting independence, supporting recovery and wellness, and reducing hospitalization.
